Facility is to have maintenance personnel inspect tubs and slow draining by 5PM today. If slow draining cannot be corrected today, rooms should not be rented until repaired.

Inspector Notes: Observed stains and holes (left bed) on yellow top bed covering in Room 403. Observed ripped mattress pad cover in Room 203. Any bed linens having excessive stains, rips, or holes are to be taken out of service. Observed desk/office chair provided in Room 203 to be almost completely stripped of vinyl covering and exposing fabric backing. Only small area of vinyl was left on chair. Chair cannot be repaired and is to be discarded and replaced with a chair in good repair. Replace chair in Room 203 within 7 days.
