Inspector Notes: Observed soiled nonfood contact surfaces of fan on floor in kitchen, rolling carts, shelves, coolers, and freezers. PIC will have all items cleaned within five to seven days.

Inspector Notes: Observed excessive ice build up on the inside walk in freezer. Door was frozen shut. Employee had to open door for EH. PIC stated they had it serviced in the past and will contact the person tomorrow to look at it again.
