Inspector Notes: Room 103: Observed bathroom had not been cleaned since last guest. Observed several old french fries on bathroom floor near tub and behind/side of toilet. Observed piece of bar soap in tub drain. Room 103 is not to be rented to guests until room and bathroom has been cleaned to sight and touch. Room 206: Observed dirt or debris on sink vanity. Appears that vanity had not been cleaned since last guest. PIC is to have housekeeping clean sink vanity to sight and touch by 12/7/22, 5:00 PM. Room 215: Observed large hole in tub where faucet plumbing exits tile wall and faucet does not cover hole, leaving large exposed gap/hole. PIC is to ensure large gap/hole is caulked or filled by 12/13/22, 5:00 PM.

Room 212: Observed sticky spots on floor in addition to dirt and debris on floors in Room 212. PIC is to ensure floor is mopped and not sticky by 12/07/22, 5:00 PM. Room 117: Observed large hole (6″ Diameter) in wall, near floor, of left bed. PIC is to ensure large hole in wall is to be repaired and painted by 12/13/22, 5:00 PM. Rooms 116, 117, 206, 212, and 215: Observed lamps provided in rooms not in good repair. Room 116, lamp missing bulb. Room 117, no knob provided to turn on dual lamps at nightstand. Room 206, lamp missing lamp shade. Lamp shade at night stand excessively bent and cracked. Room 212, lamp at night stand missing bulb and missing push button on/off switch. Room 215, lamp at night stand has blown bulb or is in operable. PIC is to ensure the following corrective actions are taken within the specified timeframes. Room 116: Replace missing light bulb, 12/7/22, 5:00 PM. Room 117: Replace missing knob to turn lamp on/off by 12/7/22, 5:00 PM or replace lamp by 12/13/22, 5:00 PM. Room 206: Replace missing lampshade and bent/cracked lampshade by 12/7/22, 5:00 PM. Room 212: Replace lamp at nightstand for guest use. Room 215: Replace blown light bulb at nightstand by 12/07/22, 5:00 PM.

Room 206: Lamb missing lampshade at night stand and excessively bend and cracked. Room 212: Lamp at nightstand missing bulb and missing push button on/off switch. Room 215: Lamp at nightstand has blown bulb or is inoperable. PIC is to ensure the following corrective actions are taken within the specified timeframe: Room 116: replace missing light bulb by 12/07/22, 5:00 PM. Room 117: Replace missing knob to turn lamp on/off by 12/07/22, 5:00 PM or replace lamp by 12/13/22, 5:00 PM. Room 206: Replace missing lampshade and bent/cracked lampshade by 12/6/22, 5:00 PM. Room 212: Replace lamp at nightstand for guest use. Room 215: Replace blown lightbulb at nightstand by 12/7/22, 5:00 PM. Rooms 106, 116, 212, and 215: Observed missing, inoperable, or not secure smoke detectors in rooms noted. PIC is to ensure the following corrective actions are taken within the specified timeframes. Rooms 106 and 116: Smoke detector not provided and missing from room. Rooms 106 and 116 are to be closed and taken out of rotation until working smoke detectors are provided in rooms. Room 212: Smoke detector chirping indicating low battery. Room 212 is to be closed and taken out of rotation until battery is replace or working smoke detector is provided in room. Room 215: Smoke detector is hanging off wall and not properly secured to base attached to wall. Room 215 is to be closed and taken out of rotation until smoke detector is properly secured to wall or replaced as needed to provide operable smoke detector in room. Room 117: Observed busted dresser drawers. When opened, dresser drawers fall out of dresser. Repeat: PIC is to ensure furniture provided in rooms for guest use is in good repair. PIC is to ensure dresser in Room 117 is repaired or replaced by 12/13/22, 5:00 PM. Rooms 206 and 212: Observed excessive rust and bubbling/rust areas on refrigerators and microwaves provided in guest rooms. They are to be cleaned to sight and touch and in good repair. PIC is to ensure rusty refrigerators and microwaves in Rooms 206 and 212 and all other rooms throughout facility are replaced by 01/06/23, 5:00 PM. Room 206: Observed unknown green leakage in refrigerator (freezer section and bottom). Refrigerator furnished for guest use has not been cleaned since last guest use. PIC is to ensure refrigerator is clean to sight and touch by 12/07/22, 5:00 PM. Rooms 103, 206, and 212: Observed box springs, mattresses, mattress pads, and sheets provided by facility not in good repair. PIC is to ensure following corrective actions are taken within specified timeframes. Room 103: Observed left box spring having tears and excessive holes in covering. Box spring is to be replaced by 01/06/23, 5:00 PM. Room 106: Room with suspect bedbug. Observed torn bed mattress separating at seam right foot corner of mattress. Separation at seam is approximately 24″-30″ in length. PIC is to ensure mattress is replaced before room can be reopened for guest. PIC is to call EH for inspection before room can be reopened for guest. Room 206: Observed excessive holes and tears in mattress. PIC is to ensure mattress is replaced by 01/06/23, 5:00 PM. Room 212: observed holes (appears to be cigarette burns) in sheets provided for guest use. PIC is to ensure sheets in Room 212 are changed and discarded in Room 212 by 12/07/22, 5:00 PM. PIC is to ensure all linens used throughout facility having excessive holes, tearing, etc. are to be discarded and replaced by 01/06/23, 5:00 PM. Room 212 or 215: Observed torn bed mattress separating at seam top left corner of right bed. Separating at seam is approximately 24″-30″ in length. Room is to be closed. PIC is to ensure mattress is repaired before room can be re-opened for guest. PIC is to call EH for inspection before room can be re-opened for guest.


28 – beds, mattresses, mattress pads, springs, frames, pillows, and bed coverings clean; vermin free and in good condition; bed linen properly supplied, changed and laundered

Inspector Notes: Room 106: Observed what appeared to be a dead bedbug in mattress on right bed, left corner at food of bed. Did not observe any other bugs dead or alive. Out of caution, Room 106 including adjacent Rooms 104 and 108 are to be closed until rooms can be inspected by a licensed pest control professional and a course of corrective action can be determined. PIC is to contact pest control company for inspection by 12/07/22, 5:00 PM. Pest control professional is to perform inspection of Rooms 104, 106, and 108 by 12/8/22, 5:00 PM and report findings to EH within that timeframe. Rooms 104, 106, and 108 are to remain closed until they are determined to be free of pests and EH called for reinspection. Reinspection can be up to 48 business hours since notification for reinspection.


31 – barrier adequate, gate locked, and installed properly; warning sign posted, procedures and log maintained
