Inspector Notes: Observed case of raw beef and raw ground beef stored over RTE (Ready to Eat) foods: gallon jugs of buttermilk and whole milk. Can not store raw meat over RTE products. Items in the WIC on shelves. COS: PIC removed all items and properly stored on shelves.

Inspector Notes: Observed soiled food contact surface of automatic ice machine in kitchen. Observed black mildew on the inside of unit. PIC will have unit cleaned today. Do not serve ice to customers. Get ice from an approved source.

Inspector Notes: Observed soiled nonfood contact surfaces of shelves, freezers, fans, etc. throughout kitchen. PIC stated he will have all equipment cleaned within five to seven days. Encouraged after hours deep cleaning.

Inspector Notes: Observed hot water not turned on in the men’s and women’s restrooms. Need to have hot water operating by 08/04 or establishment will be closed. Also observed sink in men’s restroom loose from wall. Repair by 08/04.

Inspector Notes: Observed food debris on kitchen floors, walls, and ceilings. PIC will have surfaces cleaned within five to seven days.
