Inspector Notes: Repeat Violation: Observed soiled food contact surfaces of Magic Chef microwave and automatic ice machine. Observed mildew on the inside of ice machine and a build up of debris inside the microwave. CFSM/PIC will have all items cleaned today. Do not serve ice to customers.

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods above 41° in coolers (Prep Cooler and Hosizaki 2-Dr Unit) located in kitchen. See temperature log for said items. TCS foods must be cold held at 41° or below. COS: CFSM/PIC discarded all items.
