Inspector Notes: Observed bedbug infestation in Room 112 and suspect bedbug infestations in rooms 106, 108, 118, 208, and 212. Observed permit holder or person in charge self-treating suspect bedbug rooms with suspect pesticide powder (sprinkled under bed mattresses, on top mattress under mattress pads, along floor/wall juncture behind beds, under beds, along bedframe/floor junction. Permit holder or person in charge is to immediately stop unlawful self-treatment of bedbugs or pests in rooms 106, 108, 118, 208, 212 AND ANY OTHER AREA OR GUEST ROOM WITHIN FACILITY. PARTIAL ROOM CLOSURES OR RESTRICTED ACCESS NOTED BELOW Rooms 106, 108, 112, 118, 208, and 212 with bedbugs, suspect bedbugs and suspect pesticide powder are to be CLOSED IMMEDIATELY AND NOT OCCUPIED BY NEW GUESTS. Adjacent rooms, 104, 106, 110, 206, and 210 are to be CLOSED IMMEDIATELY AND NOT OCCUPIED BY ANY NEW GUESTS. If any room noted above has an existing guest currently in the room (including Room 112), there is a extremely high chance of transferring bedbugs to another room if the guest is moved. Current guests are allowed to stay in the room until further advise/guidance is provided by licensed pest control professional as to treatment of the rooms. Permit holder or person in charge is required to contact a professional pest control operator for treatment of bedbugs in rooms stated above by 8/9/22, 5:00 PM. Permit holder or person in charge is to have pest control professional provide EH with a type written, detailed, concise plan of corrective of action by 8/10/22, 9:00 AM. Corrective action plan will be reviewed and approved by EH prior to ANY work being performed. After work/treatment is complete, professional pest control operator is to provide EH with a detailed scope of work performed, as well as any further actions to be taken by the permit holder or person in charge. Prior to rooms being reopened for guest use, permit holder or person in charge is to contact EH to schedule a re-inspection. Failure to comply with the actions noted above may led to enforcement actions being taken, which may include suspension or revocation of the permit holder’s Tourist Accommodation Operational Permit.

Inspector Notes: Room 121: Observed excessive leak/dripping under vanity sink with 5 gallon bucket under sink to catch water. Toilet would not operate and flush. Room 121 is to be closed as an available, rentable room due to the above violations and is not to be rented until violations have been corrected. Room 108: Observed shower curtain rod is bent and not functional as intended. Shower curtain rod is to be replaced. Room 108: Observed bathroom door with hole busted in door. Busted door is to be repaired or replaced. Room 110: Observed bathroom door with prior patched hole in door. Patched area is not smooth to touch and easily cleanable. At least two layers of paint are peeling off door. Door is to be repaired or replaced so that it is easily cleanable, smooth to the touch, and painted. Room 113: Observed large areas of ceiling texture above shower has peeled off ceiling. Ceiling texture is to be repaired or ceiling texture scraped off and ceiling painted. Room 113: Observed blown light bulb in vent/light combo above shower causing poor lighting in restroom. Replace light bulb or light fixture. Inspector Notes: All rooms inspected showed evidence of lack of cleaning by house keeping staff. Excessive dust, trash and debris noted on floors, on or behind furniture, behind doors, window sill, stains on walls, etc. EH expectation is that permit holder or person in charge is to properly instruct and retrain house keeping staff by August 11, 2022, 12:00 PM. Permit holder or person in charge is to be provide EH with a housekeeping guidance document that outlines expectations of house keeping for routine cleaning of rooms between guests (what to do/clean) as well as a heavy cleaning schedule , by August 11, 2022, 12:00 PM. Going forward all rooms cleaned by house keeping staff are to be clean to sight and touch prior to guest changes and a deep cleaning at least once per month. Room 106: Observed patched wall where what appears to be a previous large hole in wall. Wall is poorly patched with rough, not sanded spackling and different painted sheetrock. Permit holder or person in charge is to repair wall so that it will be finished smooth to touch and repainted to existing room color. Room 124: Observe large hole (~18 x 24 inches) in shared wall between bath and bed. Room 122:Observed both night stand lamp shades busted with other covering fabric material separating from shade. EH opinion is the lamp shades cannot be repaired. Observed corner floor lamp without lamp shade. Permit holder or person in charge is to replace night stand lamp shades not in good repair and install lamp shade on floor lamp missing shade. Room 113: Observed left over food in mini fridge. Permit holder or person in charge is to ensure food is immediately removed from mini fridge and fridge is sanitized and clean to sight and touch. Room 121: Observed two dresser drawers busted and will not easily close. EH opinion is that dresser cannot be repair to function and operate as intended. Permit holder or person is charge is to repair or replace faulty dresser or dresser drawers. Drawers also showed bugs and other debris, evidence that the drawers have not been vacuumed or cleaned in some time. Inspector Notes: All rooms inspected showed evidence of worn and thin linens, stained linens, bedspreads, and mattress pads, linens, bedspreads, and mattress pads with cigarettes burns, rips, tears and similar. Permit holder or person in charge is to go through all lines, bedspreads, and mattress pads and discard and linen, bedspread, or mattress pad that has large and or multiple staining, rips/tears, multiple burns, etc. Linens, bedspreads, and mattress pads provided for guest use are to be in good repair. EH expected date of correction is August 15, 2022, 12:00 PM, unless permit holder or person in charge provides EH with a written plan of corrective action which requests an expected longer timeframe for correction. Room 113: Observed mattress have large rip (18+ inches) along left seam and not in good repair. EH opinion is mattress cannot be repaired and is to be replaced. Inspector Notes: During inspection it appeared that facility is offering “extended stay” for guests and guests are using some rooms as permanent accommodations. It appears that typical motel/hotel services are being provided to these rooms/guests. Room 112 is not being serviced by facility. Person in charge stated that house keeping is instructed not to enter the room. Another guest I spoke with during inspection has been staying at motel for approximately one month. Permit holder or person in charge is to provide EH with written documentation as to whether they are operating a motel or extended stay operation OR which rooms are to be exclusively used as extended stay. Rooms NOT designated as extended stay CANNOT be used as a permanent accommodations and facility is required to provided basic services to these rooms. Once rooms are designated as extended stay they are NOT to be used for typical overnight travel guest accommodations. Permit holder or person in charge is to provide the requested extended stay documentation to EH by August 15, 2022, 12:00 PM, unless permit holder or person in charge provides EH with a written request for a longer timeframe.


34 – grounds, walkways, porches and hallways maintained; drives provide unobstructed width and proper spacing; graded to drain; trailer spaces marked clearly, areas clean and maintained in good repair

Inspector Notes: During inspection, observed walkways, hallways, and stairwells littered with chairs, old A/C unit, toilet broken into pieces, inoperable floor lamp, busted dresser missing drawers, old metal desk, microwave, inoperable dryer, room decor/framed pictures, guest clothing, mini fridge, and similar items. Permit holder or person in charge is to ensure all broken items, unused equipment, furniture, inoperable items, etc. are to be removed from the premises. Inspector Notes: During inspection, observed facility’s grounds and landscaping and is not being maintained as required to maintain an attractive appearance for the facility and prevent harborage and breeding of vectors and vermin. Fence is overgrown with weeds and vines. Grass and shrubbery has not been cut or maintained and is overgrown. Observed dumpster area littered with trash, baby toys, and other items. Observed multiple trash cans around facility full of trash and in need of removal. Permit holder or person in charge is to ensure grounds and landscaping are maintained to prevent harborage of vectors and vermin, and all loose trash, litter, and other items are removed from the premises.
