Inspector Notes: Observed that emergency phone appeared to be unplugged in phone box; back of phone plugs into phone line receptacle. Phone was hanging loose within box. Plugged phone into receptacle. No dial tone. Phone was not in working order at time of inspection. Emergency phone is to be repaired and capable of calling 911 directly prior to reinspection and opening swimming pool.

Inspector Notes: Performed water testing and observed pH of pool was 8.0 or greater at time of inspection. pH is to be lowered to a acceptable range of 7.2-7.8 prior to calling EH for reinspection to open pool.

Inspector Notes: REPEAT. Observed life-saving throw ring scattered on pool deck near emergency phone. life ring did not have required throw rope attached to ring at time of inspection. Replace thrown rope on life ring (rope to be minimum 1/4″ diameter and 1.5 times the width of the pool) and place life ring in a conspiquious area where it can be used at the time of an emergency.

Inspector Notes: At time of inspection, facility could not produce a copy of current CPO’s certification certificate. A copy of the CPO’s current certification certificate is to be maintained onsite and review at any time. Obtain copy of CPO’s certification certificate immediately.

Inspector Notes: It does not appear that CPO is making minimum number of weekly visits to oversee pool. Facility manager could not produce any documentation of CPO visits. CPO is required to make a minimum of two weekly visits or as many visits as necessary to ensure pool is operating in complaince with Pool Rules and pool is safe for guest use. CPO is to begin making required visits immediately.

Inspector Notes: At time of inspection, facility could not produce any water testing/sampling, pool adjustments, daily inspection/self-check records, or monitoring records. Daily Records are to be maintained by CPO and designated responsible person and are to be available for review at all times.

511-3-5-.22(12) – (12) a safety self-inspection shall be conducted daily by the trained operator or responsible person and documented on a log sheet.

Inspector Notes: Facility has not appointed a responsible person to perform daily pool monitoring and water quality checks in the absence of CPO. Responsible person is to be trained in basic pool operations and water quality testing by CPO. Responsible person is required to test water quality as required by Pool Rules and perform any other requirements within the Pool Rules in the absence of the CPO. Facility is to immediately assign and begin training a responsible person for pool maintenance and monitoring.

Inspector Notes: At time of inspection facility could not produce or find any type of pool test kit for testing water quality. Every pool is required to have an accurate and reliable test kit capable of testing pool water chemistry/quality as prescribed in Pool Rules. Obtain and begin using proper test kit immediately.
