Inspector Notes: Room 211–Observed evidence of dead bugs/roaches behind left bed and collected inside refrigerator near drain. Observed small live bug between mattress and box spring cover. All furniture in room is to be moved and thoroughly cleaned behind and under all furnishings in room. Dead roaches are to be removed from refrigerator. Refrigerator is to be cleaned to sight and touch and sanitized by 5:00 PM, 04/07/22, or before room is rented to next guest. Room and all furnishings are to be cleaned by 5:00 PM.

Inspector Notes: Repeat Violation: Current inspection report 11/29/21 was not posted as required. Inspection report had been removed from wall most likely due to score of 77. Current inspection was posted prior to leaving facility.

Inspector Notes: Repeat Violation: Room 115–Observed shower pan in room extremely dingy and dirty looking. Have previously observed dirty and dingy shower pans in various rooms. Shower pan in Room 115 and any other rooms is to be cleaned to sight and touch. Cleaning may require scrubbing shower pan. Room 112–Observed toilet base very dirty. Evidence of build up and what appears to be dried urine covering right side of toilet. Toilet is to be clean to sight and touch by 5:00 PM or before room is rented to next guest.

Inspector Notes: Repeat Violation: Room 208–Observed water damaged and busted entertainment center/door. Room 215–Observed water damage and busted entertainment center. Refrigerator drain appears to be stopped up. Water puddled in front of refrigerator when door was opened. Damaged rooms during previous inspection. Repair or replace damaged entertainment centers in Rooms 208 and 215 and any other rooms within 30 days. Room 219–Observed closet door very dusty indicating closet area has not be cleaned in some time. Evidence of where broom has swept around items in closet, but not moved items to clean under and behind items.
