Inspector Notes: Food service inspection shows that the CFSM has not properly trained employees on food safety responsibilities. CFSM is not monitoring employees to assure proper handling and preparation of food and TCS (Temperature Controlled for Safety) foods. PIC stated that the employees will all be properly trained on food safety.

Inspector Notes: Observed no handwashing supplies (soap) at handwashing sink used by employees and located in the kitchen. COS: PIC installed hand washing supplies (soap).

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods above 41° in the Artic Air Unit #2. See temperature log for said items. COS: All items discarded. Will use Artic Air Unit #1 until Unit #2 is operating properly.

Inspector Notes: Observed several live small insects in the serving area on the counter above the small refrigerator. Can see bugs coming up from a hole in the counter to the top area. PIC stated Arrow Pest Control Company is coming today. She will have them address the issue with the small insects.
