Inspector Notes: Observed employee drink in kitchen on food prep table. No lid or straw on/in cup and cup on prep table during food preparation. COS: Employee removed drink properly stored away from food prep station.

Inspector Notes: Repeat Violation: Observed grease build up on shelves, fryers, radio, and food storage containers such as flour container. Observed a lot of shelves rusted. Also Whirlpool freezer rusted. PIC will clean equipment by 11/24/21. Also repaint freezer and shelves within 30 days.

Inspector Notes: Repeat Violation: Observed grease buildup on the walls and ceiling and throughout kitchen from the deep fryer. Observed grease and debris under and behind shelves and equipment. PIC will have floors and walls cleaned by 11/30/21.
