Inspector Notes: Observed soiled nonfood contact surfaces in kitchen, prep, and storage areas at time of inspection. Excessive build up and splatter noted on cooking equipment, shelving, drink machines, etc. Discussed with PIC all nonfood contact surfaces to be cleaned to sight and touch within 3 days of inspection.

Inspector Notes: Observed hot water at hand sink #2 in kitchen cut off. PIC cut hot water off at sink due extremely bad plumbing leak at sink. PIC cut hot water on at sink to show hot water available but was cut off due to bad plumbing leak. Owner/Pic stated he was in the process of repairing the hot water supply to the hand wash sink. Sink to be repaired and hot water available at sink by 10/23/20.

Inspector Notes: Observed hot water at hand sink #2 (rear most sink) in kitchen cut off. Owner/Pic cut hot water on at sink to show extremely bad plumbing leak at sink. Owner/Pic stated he was in the process of repairing the hot water supply to the hand wash sink. Sink to be repaired and hot water available at sink by 10/23/20. PIC is to instruct all employees to use front hand wash sink until hand wash sink #2 is repaired and has hot water available.

Inspector Notes: Observed soiled floors throughout facility during inspection. Observed food and other debris under shelving and equipment in kitchen and prep areas. Discussed with PIC need for deep clean throughout establishment. PIC is to have floors clean to sight and touch within 3 days of inspection.

Inspector Notes: Observed areas in kitchen and near mop sink with coved tile at floor and wall junction is cracked and/or missing. Observed FRP wall covering near mop sink loose at floor causing crack/gap at floor/wall juncture. Wall coving throughout establishment is to be repaired or replaced as needed within 30 days of inspection. Wall coverings throughout establishment are to be repaired or replaced as needed within 30 days of inspection to ensure sealed wall/floor junctures.

Inspector Notes: Observed dead insects under and behind drink machine and behind milkshake machine, cooling units and under shelving in kitchen,storage and prep areas. PIC to clean and sanitizer all areas where dead insects were located within 3 days from date of inspection.
