Inspector Notes: Observed soiled food contact surfaces of the inside of microwave in kitchen, and storage area used to store knives between uses. In between uses in the kitchen, knives are stored in between the True prep unit and prep table (dirty). PIC will have microwave cleaned today and removed knives.

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods above 41°F in True prep unit. TCS food shall be held at 41°F or below. See temperature log for said items. COS: PIC discarded all the food.

Inspector Notes: Observed soiled nonfood contact surfaces of the True coolers, prep coolers, freezers, rolling carts, shelves (rusty) and warmers–Nemco, Winston, etc. PIC will have everything cleaned by 09/28/19.

511-6-1.07(5)(a),(b) – good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)

Inspector Notes: Observed soiled floors and walls throughout the kitchen area. Observed debris under and behind equipment and shelves. Observed back door in kitchen in poor condition. The door is damaged and does not close properly. PIC will have floors cleaned by 09/28/19 and will address the damaged door within two days. It is set up to be repaired.
