Inspector Notes: Repeat Violation: Observed soiled food contact surface of Ice O’matic ice machine with black mildew/slime on the inside of unit. Observed soiled food contact surface of Panisonic microwave soiled on the inside. CFSM stated both items will be cleaned by 07/13/22. Do not service ice to customers.

Inspector Notes: Observed soiled/dirty nonfood contact surfaces of shelves, flour container, coolers, fryers, hot holding units, drink cup holder, etc. In need of a deep cleaning. Will have five to seven days to have all items cleaned.

Inspector Notes: Observed plumbing leak at 3-compartment sink in kitchen. Observed a plastic tub on the floor under the sink to catch water. Plumbing leak to be corrected within three to five days.

511-6-1.07(5)(a),(b) – good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)

Inspector Notes: Observed soiled floors and walls under and behind equipment: coolers, ice machine, drink machines, and fryers. A deep cleaning is need throughout kitchen area. Observed debris under equipment also. Will have five to seven days to clean floors and walls.
