Inspector Notes: Repeat: Subway of Bainbridge #2 still does not have a CFSM (Certified Food Safety Manager) at this facility. Subway is to contact EH to discuss corrective action and also to set up a time for an informal hearing.

Inspector Notes: Repeat: Observed TCS (Temperature Controlled for Safety) foods held above 41°F in the Duke Unit on front serving line. TCS food must be cold held at 41°F or below. See temperature log for said items. Per PIC, all items were put on serving line at 7:00 AM. EH checked temperatures between 10:40 AM and 11:05 AM. Subway does not use Time as a Public Health Control. PIC stated she turned unit down. Did not monitor temperatures. COS: PIC discarded all of the food. Stated she will monitor the unit from now on.
