Inspector Notes: CFSM (Certified Food Safety Manager) shall train all employees in their job duties to assure food handling and safety is met. CFSM is not handling responsibilities. Will retrain.

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods above required temperature of 41°F. TCS foods must be held at 41°F or below. See temperature log for said items. COS: All items discarded.

Inspector Notes: Observed improper thawing of raw shrimp and fish on food prep table in kitchen. Food should be in meat sink in a colander under running water or in a refrigerator. COS: PIC/CFSM removed the raw fish and shrimp.

Inspector Notes: Observed food debris on the inside of the Polar and Cold Tech refrigerators and freezers, observed a buildup of debris. PIC will have cleaned within three days.
