Repeat: No

Inspector Notes: Observed no hand drying provisions (paper towels) a hand washing sink in the kitchen and used by employees. No paper towels at hand sink. COS: PIC installed paper towels.


6-1A – proper cold holding temperatures

511-6-1.04(6)(f) – time/temperature control for safety; cold holding (p)

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods held above 41° in two coolers. See temperature log for said items. TCS foods must be held at 41° or below. COS: PIC discarded all items and stated that they have ordered new units and will us an ice bath until coolers have been installed. Also, the food will be monitored.


12C – wiping cloths: properly used and stored

Inspector Notes: Observed several wet wiping cloths laying on food prep tables and not stored in sanitizing solution between uses. COS: Employee removed all wet wiping cloths.

511-6-1.07(5)(a),(b) – good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)

Inspector Notes: Observed soiled floors and walls throughout kitchen area. Observed missing cove molding throughout kitchen area. Also observed the back door to the kitchen in poor condition. PIC stated they are deep cleaning 07/04/21 and repairing missing cove molding also.

Inspector Notes: Observed a lot of flies in the kitchen and dining room area. PIC stated Terminix is coming 07/01/21 and they have a plan to address the fly issue.
