Inspector Notes: Repeat Violation: Observed a lot of black mildew/slime build up inside of the Ice-O-Matic ice machine. Showed it to the PIC and he will have it cleaned by end of the day. Do not serve ice to customers.

Inspector Notes: Observed cold holding TCS (Temperature Controlled for Safety) foods above 41° in walk in cooler and in metal pans on table by flat grill. TCS foods must be cold held at 41°F or below. See temperature log for said items. COS: PIC discarded all of the food and is having WIC looked at on 11/18/25.

Inspector Notes: Observed damaged door at employee bathroom. Observed damage on the back side door in kitchen. Also holes in walls and ceiling. Observed soiled/dirty walls in kitchen dishwashing station. PIC will have floors and walls cleaned within three to five days. PIC will discuss with owner on a plan of corrective action for the damage observed during inspection.
