Inspector Notes: Observed soiled food contact surfaces of pizza screens, pans, and damaged/cracked plastic containers. CFSM/PIC will have plastic containers replaced and have encrusted deposits removed from all pizza pans and screens within three to five days.

Inspector Notes: Observed soiled floors and walls throughout the front area and back area, prep station, WIC area, etc. Floors and walls are soiled/greasy. Also debris under coolers, freezers, shelves, equipment, etc. Will have floors and walls cleaned within five to seven days.
