Inspector Notes: Observed a large bag of pork patty sausage thawing/laying in the hand washing sink in kitchen. Can not use hand washing sink for any other purpose but washing hands. COS: Employee removed the large bag of patty sausage.

Inspector Notes: Observed soiled nonfood contact surfaces of rusted shelves, inside coolers and freezers, microwave, etc. Will have everything cleaned within three to five days and will address rusted shelves.

Inspector Notes: Observed holes in the walls in the kitchen area and observed tape on the floor where damaged in kitchen area and sandwich prep station. Plans are in the works to do repairs. Materials are already on site. Plan to start repairs in January.
