Inspector Notes: Repeat Violation: Observed soiled food contact surface of Hoshizaki ice machine. Observed black mildew inside of the ice machine. CFSM cut off ice machine and stated it will be cleaned tonight. Do not serve ice to customers. EH will come back on 12/29/22.


6-1B – proper hot holding temperatures

Inspector Notes: Observed hot holding TCS (Temperature Controlled for Safety) foods in Winston Warmer below 135°F. TCS foods must be hot held at 135°F or above. See temperature log for said items. CFSM removed and discarded items. Will have the unit looked at before using again.


14B – utensils, equipment and linens: properly stored, dried, handled

Inspector Notes: Observed wet stacking metal pans on shelf against the back wall. Utensils have to be air dried before stacking. COS: CFSM had employee separate all the pans to air dry properly.

Inspector Notes: Observed soiled nonfood contact surfaces of rolling carts, shelves, Magali unit, equipment (fryers, grill warmers, coolers, etc.). All of the equipment is in need of a deep cleaning. Will have all items cleaned within 7 to 10 days.

511-6-1.07(5)(a),(b) – good repair, physical facilities maintained; cleaning, frequency & restrictions, cleaned often enough to keep them clean (c)

Inspector Notes: Observed soiled walls and floors throughout kitchen area, fry station, dishwashing area, under cooking equipment, shelves, and coolers. CFSM will have floors and walls cleaned within 7 to 10 days.
