Manager stated Arrow Exterminators Pest Control treated room on 11/23/2021. Pest control is to return and treat room again on 12/01/2021. During inspection of Room 229, observed one dead bedbug on sheets near headboard. Reviewed pest control records on file since previous inspection. Arrow Exterminators treated Rooms 229, 321, 306, and 309 for bedbugs on 10/29/2021. Inspected adjacent Room 227. No evidence of bedbugs noted at time of inspection. Room 229 cannot be rented until released by pest control company and EH. Call EH for reinspection.

Manager scheduled gas fired inspection with Watson for 11/30/21 at 9:00 AM. Hampton Inn is to forward gas fired inspection report to Environmental Health after inspection is performed. Provided EH fax number to manager to send report to add to file.

Inspector Notes: Repeat Violation: Room 114: Observed cracked shower wall. Observed handicap grab bar missing from shower wall; holes where bar mounts are evident and open. Shower pan is extremely dingy and dirty looking. Housekeeping PIC stated that shower pan is clean and the hotel has tried multiple cleaners but shower pans will not look clean. Room 116 and Room 118: Same issues observed with shower pans as Room 114. Dingy/dirty mildew buildup noted along shower junctions. Room 114 is to be taken out of rotation to be rented until shower issues noted above are repaired or shower can be replaced. Facility is to call EH when Room 114 is ready to be rented. Rooms 116 and 118 are to have mildewed caulking removed, and replaced or cleaned to sight and touch within seven days.

Room 223 was observed extremely dirty baseboards in room, especially under night stand. Housekeeping PIC stated all rooms are being deep cleaned by housekeeping staff person. PIC noted dirty baseboards and stated they will be cleaned to sight and touch by housekeeping staff person.

Inspector Notes: Repeat Violation: Rooms 116, 214, and 223 observed no mattress pads on beds that were clean and to be rented to guests. Housekeeping PIC contacted housekeeping staff to install mattress pads on all beds. PIC stated these noted rooms and put out of order today until mattress pads are installed.
