Inspector Notes: Observed soiled food contact surface of Hoshizaki automatic ice machine in kitchen. Observed black mildew on the inside food contact surface of the ice machine. PIC will have it cleaned today.

Inspector Notes: Observed food and packages/boxes sitting on the floor in back room area. Food and supplies must be 6″ off of the floor. PIC removed all items and stored them properly.
