Inspector Notes: Observed a large pan of cooked fried chicken sitting in 3-Dr cooler and uncovered to protect if from sources of contamination. Food must be covered for protection. COS: CFSM covered chicken. Observed a large container of raw ground beef in WIC in back kitchen and uncovered to protect from sources of contamination. COS: CFSM covered the large container of raw ground beef.

Inspector Notes: Observed soiled nonfood contact surfaces of all the coolers and freezers (inside and outside), fryers, fans, shelves, hot holding units, Winston Auto Sham, etc. General deep cleaning needed. PIC stated it all will be cleaned within five to seven days. Plans are already in the works.

Inspector Notes: Observed soiled floors, walls, and ceilings throughout the restaurant, in kitchen and dining room. Observed stained ceiling tiles in the kitchen area. Walls in kitchen are soiled and stained. Also missing floor tiles. CFSM stated they are already working on a facility upgrade for walls, floors, and ceilings. Have started painting on the inside and outside.

Inspector Notes: Observed several live flies in the kitchen and dining area. Observed two flies on the buffet. CFSM will call the pest control company tomorrow to start a corrective action plan.
