Inspector Notes: Repeat Violation: Observed soiled food contact surface of Hoshizaki automatic ice machine in the lobby and used by customers. Observed mildew inside the food contact surface where the ice is made. Soiled food contact surface. PIC/CFSM turned off the ice machine and will not use it until it has been properly cleaned and sanitized.

See temperature log for said items. PIC/CFSM removed and discarded all out of temperature items. Will not use WIC to cold hold any TCS foods until it is maintaining 41° or below.
