Inspector Notes: Observed two packs of ranch dressing laying on top of a large box of lettuce. The large box of lettuce was opened and the ranch dressing was laying inside of it. COS: CFSM removed the ranch dressing and removed the top layer of the cut lettuce.

Inspector Notes: Observed soiled nonfood contact surfaces of screen monitor, toasters, fryers, coolers, waffle irons, shelves, etc. CFSM stated they will have everything cleaned within three to five days.
