Inspector Notes: Observed soiled food contact surface of Hoshizaki automatic ice machine in the kitchen. Observed a build up of black mildew and calcium on the inside area. PIC will have it cleaned within 24 hours. Use ice for customers from another source.

Inspector Notes: Observed soiled nonfood contact surfaces of the large oven, fryers, shelves and storage containers. Also some shelves in back are rusting. Must be resurfaced, painted, or replaced. PIC will have everything cleaned up with 7 days. Rusting shelves will be repainted or replaced.

Inspector Notes: Observed soiled floors up and under shelves and equipment. Observed a lot of dust build up and debris along window area and wall at the fry station, prep station in kitchen, and middle area. PIC will have floors and walls cleaned within 7 days.
