Inspector Notes: CFSM (Certified Food Safety Manager) is not supervising/instructing food service employees in the techniques of sanitary food handling and proper maintenance of the facility. CFSM is not ensuring all employees handle any unpackaged food in a safe way. CFSM will retrain employees today.

Inspector Notes: Repeat Violation: Observed employee not wearing gloves picking up onion rings and putting them on a plate to serve customers. Employee using bare hand to pick up onion rings. Must wear gloves or use an alternative method such as tongs or paper, etc. Cannot touch with bare hand ready to eat food to be served. COS: Explained to employee procedure. Employee discarded onion rings.

Inspector Notes: Observed black slime/mildew on the inside food contact surface of the Manitowoc automatic ice machine in kitchen. Cannot serve ice to customers. Ice machine will have to be cleaned today before serving ice.

Inspector Notes: Observed soiled shelves, coolers, dishwasher, stove, hot holding units, etc. Equipment and shelves are soiled/dirty. Some have a build up of debris/grease. PIC will have all items cleaned within seven to ten days.

Inspector Notes: Observed soiled floors and walls throughout kitchen area. Observed debris/grease buildup under and behind equipment and shelves. Walls are soiled. PIC will have floors and walls cleaned within seven to 10 days.
